Admissions Forms

On this page you will find various forms used during and after the application process.  There are also links to these forms in our Admissions information, or Academics.  Here, they are all in one place for your convenience.


Every student must complete an application.



Once you have been accepted, your Letter of Intent (Link below) must be returned to FST.  It indicates to us that you will study at FST and when you wish to begin.  It also tells us if you decline to study at FST, or if you wish to defer your acceptance to a later semester.  A copy is sent with your Acceptance Letter.

You are not an official student of FST until this form is received.


You cannot apply for Financial Aid from FST or Federal Student Aid before you are accepted.  But, you may want to prepare the forms and begin the process in advance. This will foster a faster determination of possible financial aid for you when it is time (See Financial Aid for specific details).  Here are the forms:

Return the appropriate and completed form to the Director of Recruitment at FST.


Catholic Employee Discount Form

Employees of various Catholic Institutions may apply for a 25% discount or a matching fund opportunity using this form.  It must be signed by your employer.  Return this completed form to the Director of Recruitment at FST.


Federal Student Aid

  • The FAFSA (Free Application for Federal Student Aid)

The on-line FASFA application process must be completed to receive Financial Aid from FST and/or Federal Student Aid (Loans).  It is a tool used to assess your degree of need for financial aid.



After you are accepted, this form must be completed by all International Students.  Return this completed form to the Director of Recruitment at FST.
