On this page you will find links to the various forms used during and after the application process, gathered in one place for your convenience.


Every student must complete an application.

Franciscan School of Theology Application

After you are accepted, your Letter of Intent  must be returned to FST.  It indicates to us that you will study at FST and when you wish to begin.  It also tells us if you decline to study at FST, or if you wish to defer your acceptance to a later semester.  You will find a link to the Letter of Intent form below. A copy of this form is also sent to you with your Acceptance Letter. You are not an official student of FST until your completed form is received by FST.

While applicants will be notified of any financial aid award when they have been admitted to FST, you will want to begin working on your financial aid forms (FST and Federal) as soon as possible in the general application process, in order to expedite award determination if the case you are accepted.

Catholic Employee Discount Form

Employees of Catholic Institutions may apply for a 25% discount or a matching fund using this form.  Return this completed form, signed by your employer to the Director of Recruitment and Admissions at FST.

Professed Members of the Secular Franciscan Order (OFS)

FST extends financial support to professed members of the Secular Franciscan Order (OFS).  OFS applicants have two discounts available to them: 25% reduction in tuition, or up to 50% matching funds from FST when a fraternity contributes to their tuition.

Federal Student Aid

If you wish to receive financial aid you must complete the on-line Free Application for Federal Student Aid (FASFA) application. The FAFSA is used to determine both Federal Student Loan and FST Financial aid.  You will want to begin your application for financial aid as early as possible in the application process in order to expedite any financial aid award decision.

The International Students Financial Declaration form must be completed by all International Students after being accepted by FST.  Return the completed form to the Director of Recruitment at FST.
